How It Works
- You must be connected to the Internet for this form to work.
- We will customize this form with your name, contact information, and the items that you buy and e-mail it back to you. You can keep it on your computer or a disk and run it from there.
- We can change the default information on your custom order form at any time.
- Do not send your order more than once. We will confirm it within one business day.
- You may change or delete any default field in this form before you send it, only fields with information in them will be sent to us.
- We will not accept any orders without your name and contact information (Phone, Fax, or E-Mail)
- If you would like to order with a credit card this form sends your order via regular e-mail which is NOT SECURE. If you are concerned about putting this information on the web contact us and we can file your credit card. You can then just type "Credit Card Order" in the "Additional Information" box and we will look up your card number and bill your card. Your numbers will not appear anywhere.
